An investigation is underway in Michigan after a 5-year-old boy reportedly died on Friday when the hyperbaric chamber he was in exploded.

The boy was in the hyperbaric chamber at a Troy medical facility when the explosion occurred. Responding firefighters found the pressurized device — which provides oxygen therapy to patients — on fire and the boy dead at the scene, according to The Detroit News.

Metro Detroit News reported that the boy’s mother was near him at the time and was also injured in the explosion. Troy police said the fire was contained within minutes and the facility did not suffer serious damage, according to KDIV.
